How do I join the practice?

If you live within our catchment area and wish to register at this practice please ask the receptionist for details about registering.

The registration forms will ask you for details such as your name, address, date of birth, telephone number and previous address and doctor. These details are needed to help the Health Authority obtain your medical records from your previous doctor and the form needs your signature. You will also need to bring in photographic ID and proof of your current address.

After completion of this form you may be offered a New Patient Check with the Health Care Assistant. You will be given forms to fill out which cover your past medical history, regular medications and various lifestyle questions. If you have complicated medical problems the nurse may make an appointment for you to be reviewed by the doctor.

If the practice feels they are unable to accept you as a patient it will explain the reasons for this to you. The practice can decline to register patients for various reasons;these include living outside the catchment area.
