Test Results

Please note that for reasons of confidentiality we no longer give out test results at the desk. Please call for your results on 0844 477 3400 (option 5) between 2.30 and 6.00 p.m. weekdays.

Swine Flu Immunisations

Swine flu immunisations have now started at this surgery. If you fall into any of the following risk groups please make an appointment for your vaccination:

Risk groups – patients with:

  • Heart disease
  • Asthma/COPD
  • Diabetes
  • Kidney, liver or neurological problems
  • Conditions that impair the immune system
  • Carers

The vaccine is also recommended for pregnant women and people who care for patients with immune problems. Please book your immunisation at the front desk.

Sick Notes

How do I get a sick certificate?

Illness which necessitates less than 7 days off work can be self-certified. Self-certification forms are generally available from Personnel Department within companies and from the DSS.

If you are off sick for more than 7 days you will require a doctor’s certificate. Doctors can only issue an initial certificate if you have been seen during your illness by a medical practitioner (this does not necessarily have to be a doctor at the practice; it may be a hospital doctor or an out of hour’s doctor). If you require a renewal of your sick certificate please ring the surgery (after 10:30) when the certificate is due to run out. You will be asked your:

  1. Name
  2. Address and telephone numbers
  3. Dates that you require the certificate to cover
  4. Your medical condition

If the doctor has a query over your certificate they may telephone you or ask you to attend the surgery.

Sick certificates, like prescriptions, take 3  full working days to process.

If your employer insists on a certificate for the first week of illness (i.e. the normal self certified period) a private certificate can be issued. These certificates will incur a charge.

How do I get forms completed by a doctor?

Some forms require completion by a doctor. These forms may include:-

  • Private sick notes
  • Holiday cancellation forms
  • Private letters
  • Insurance claims
  • Passport applications

Completion of these forms will carry a charge as this is not part of our NHS responsibility. A list of charges is held in reception. Please leave such forms with the reception staff who will pass them on to the doctor. Please allow 2 - 3 working days for completion.
