Test Results

Please note that for reasons of confidentiality we no longer give out test results at the desk. Please call for your results on 0844 477 3400 (option 5) between 2.30 and 6.00 p.m. weekdays.

Swine Flu Immunisations

Swine flu immunisations have now started at this surgery. If you fall into any of the following risk groups please make an appointment for your vaccination:

Risk groups – patients with:

  • Heart disease
  • Asthma/COPD
  • Diabetes
  • Kidney, liver or neurological problems
  • Conditions that impair the immune system
  • Carers

The vaccine is also recommended for pregnant women and people who care for patients with immune problems. Please book your immunisation at the front desk.

How do I join the practice?

If you live within our catchment area and wish to register at this practice please ask the receptionist for details about registering.

The registration forms will ask you for details such as your name, address, date of birth, telephone number and previous address and doctor. These details are needed to help the Health Authority obtain your medical records from your previous doctor and the form needs your signature..

After completion of this form you will be offered a New Patient Check with the Health Care Assistant. You will be given forms to fill out which cover your past medical history, regular medications and various lifestyle questions. If you have complicated medical problems the nurse may make an appointment for you to be reviewed by the doctor.

If the practice feels they are unable to accept you as a patient it will explain the reasons for this to you. The practice can decline to register patients for various reasons;these include living outside the catchment area.

Catchment Area

The practice area consists of the area to the south of Churchill Way and bounded by the Ringway. Anyone residing in this area is able to register with the practice. The practice is able to take on patients from other areas in urban Basingstoke only if there is capacity  to  do so.

Patients already on the practice list who move to other areas within urban Basingstoke are able to stay registered if they wish to do so. Individuals moving outside this area need to register with a practice local to them.

The area designated 'urban Basingstoke' is recorded at the practice-please enquire.

From April 2004 the Government changed registration, you are registered with the practice rather than a particular doctor. However, you will be allocated to a particular doctor at the time of registration, and will be encouraged to see that doctor whenever possible. This helps ensure continuity of care for patients.
