There are a few different ways in which patients can request prescriptions from The Hackwood Partnership:
1) By ordering online. To order a repeat prescription online you will need to have registered for patient access. You need to request this access in person at the Practice. If you are signed up for patient access, please follow this link to request your repeat prescription.
This service is available for patients aged 11 or younger and 17 and over. (Please contact the surgery to obtain the login details for your child.)
Patients aged 12 to 16 – to request a prescription, please return to the main menu and press the ‘contact us’ button, and email your request to the practice.

2) By posting your prescription request to us and attaching a stamped return envelope for us to post the prescription back to you. Alternatively, you can ask for us to arrange for a local pharmacy to collect your prescription from us. Certain pharmacies deliver prescriptions to patients at home and you will need to arrange this with the pharmacy.
3) By faxing the request to us on 01256 357289.
4) By completing a repeat prescription form at the reception desk.
