New Patients

Registering as a new patient

Please note that patients who reside outside of our boundary are not eligible to register with Acorn Health Partnership. If you move outside of our Practice boundary, you will be asked to register at another Practice who does cover your area.

When you register with us you will need to complete a form which will ask you for details such as your name, address, date of birth, telephone number, previous address and doctor.  These details are needed to help the Health Authority obtain your medical records from your previous doctor and the form needs your signature.

You will also need to bring in photographic ID and proof of your current address.

After completion of your registration form, you will be asked to complete a questionnaire, which covers your past medical history, regular medications and various lifestyle questions.  This will be reviewed by your doctor who may request a review appointment with either themselves or one of the nurses.  You can download the questionnaire here New Patient Questionnaire 

If we are unable to accept you as a patient we will explain the reasons for this to you.

 

If you require a translator and are unable to attend the surgery with someone who can support you, please indicate this to the reception staff.
