A fit note is not needed for the first seven days of illness. Self certification forms are available from your employer or are available to complete online. Click here for more information.
If you are off sick for more than 7 days you will require a fit note. Doctors can only issue an fit note if you have been seen during your illness by a medical practitioner. This does not necessarily have to be a doctor at the practice; it may be a hospital doctor or an out of hours doctor.
If you require a fit note please ring the surgery after 10:30. You will be asked to confirm the dates that you require the certificate to cover and the reason why you require it. If the doctor has a query about your certificate they may telephone you or ask you to attend the surgery.
Fit Notes take three working days to process.
If your employer insists on a certificate for the first week of illness (i.e. the normal self certified period) a private certificate can be issued and this will incur a charge.
If you already have a fit note, and require an extension, please call and speak to the receptionists who will arrange this with the GP. Your doctor may need to call and discuss this with you further.
Returning to work
You should go back to work as soon as you feel able to and with your employer’s agreement (this may be before your fit note runs out.)
You do not need to go back to see your doctor before going back to work.
Your doctor cannot give you a fit note stating that you are ‘fit for work’.
